Instructions

Each of the applications is a PDF form, with fields to be completed by the applicant. These can be viewed using Adobe Acrobat Reader, which can be downloaded for free from Adobe's website (www.adobe.com). Use of their more fully featured products, Adobe Acrobat and Adobe Acrobat Professional, is strongly recommended as the form fields are retained when saving the forms with these products. Educational pricing is available for these products.

When a section contains only a small number of options that are exclusive to each other, radio buttons have been used (for example, is the application biomedical or Clinical and Population Health Research based?). The applicant need only click on the correct button to indicate their choice. The rest of the form fields are text boxes, where responses can be entered once the applicant has clicked on the space left for that section. The font and font-size should not be altered by applicants. If additional space is required and allowed, PDF files should be included in the "Appendix" folder as indicated at the beginning of each form section.

As each of the applications is assessed separately, each should be submitted on separate CD-ROMs. For example, if an applicant wishes to apply for both a biomedical and Clinical and Population Health Research grant each of these applications should be completed and submitted separately on two CD-ROMs. There are two notable exceptions to this rule:

  1. If the applicant wishes to apply for both an operational grant and a career development award, they should submit both Forms MSSC-1 and MSSC-3 on the same CD-ROM. Both files should be placed in the main folder as described in Structure for application CD-ROMs.
  2. When the research involves experimentation on human subjects, Form MSSC-6 should be included in the main folder as described in Structure for application CD-ROMs.

Application Forms

MSSC-1 - MSSC Research Grant Application
This form is to be completed by applicants for research grants from the Society. As indicated at the bottom of the first page of the application, the CD-ROM submitted to the Society must be accompanied by a completed hard copy of the first page, which must be signed by the applicant, their department head, and either the Dean of their faculty or the Director of their institution.

MSSC-1a - Suggested external referees
This form lists contact details of external referees suggested by the applicants for research grants. Individuals who are from the same institution or who have collaborated with the applicant in the past 3 years should not be included in this form.

MSSC-2 - Application for a Postdoctoral Fellowship
This form is to be completed by applicants for postdoctoral fellowships from the Society. As indicated by point 9 in the instructions for this application, the CD-ROM submitted to the Society must be accompanied by a completed hard copy of the second page, which must be signed by the applicant, their supervisor, and their department head. The applicant must also provide copies of Form MSSC-2a (described below) to each of three referees and ensure that the Society receives them before the submission deadline.

MSSC-2a - Confidential Assessment of Applicant for Fellowship
This form is to be supplied by applicants for postdoctoral fellowships to each of their chosen referees. When completed, the referees are to send the forms directly to the Society as explained on the form itself (the forms are not to be returned to the applicant). Both pages must be completed, signed, and sent to the Society in hard copy.

MSSC-2b - Application for a postdoctoral fellowship renewal
This form is to be completed by those already holding an MS fellowship who intend to apply for future funding.

MSSC-3 - Application for a Donald Paty Career Development Award
This form is to be completed by applicants for career development awards from the Society. As indicated by point 8 in the instructions for this application, the CD-ROM submitted to the Society must be accompanied by a completed hard copy of the second page, which must be signed by the applicant. The applicant must also ensure that Form MSSC-3a (described below) is completed, signed by the President of their institution, and submitted to the Society before the submission deadline.

MSSC-3a - Recommendation of the Nominating University for a Donald Paty Career Development Award
This form is to be supplied by applicants for career development awards to their institution. Both pages must be completed, signed, and sent in hard copy to the Society.

MSSC-4 - Application for a Research Studentship
This form is to be completed by applicants for research studentships from the Society. As indicated in the instructions on the first page of this application, the CD-ROM submitted to the Society must be accompanied by a completed hard copy of the second page, which must be signed by the applicant, their supervisor, and their department head.

MSSC-4a - Application for a Research Studentship Renewal
This form is to be completed by those already holding an MS research studentship who intend to apply for further funding.

MSSC-6 - Clinical Investigation
This form is to be completed by applicants whose research will involve experimentation on human subjects. It is to accompany another application for support from the Society as described by point 2 above. As indicated in the instructions on the first page of this application, the CD-ROM submitted to the Society must be accompanied by a completed hard copy of the second page, which must be signed by the applicant's department head and a representative from the applicant's institution.

Structure for application CD-ROMs

Application CD-ROMs submitted to the MS Society should contain one main directory labeled with the application year and applicant's name as follows: Year_Lastname_GivenNames. (ex. 2003_Smith_JohnJoe) Within this main directory, there should be (potentially) five main folders. They are:

CV
This folder contains the curriculum vitae (CV) of the applicant. The Society requests that applicants submit CV's in the Canadian Institutes for Clinical and Population Health Research (CIHR) Common CV format. Please see www.commoncv.net for more information. The Common CV website can produce a PDF format file for inclusion on the CD-ROM to the MSSC. In the case of the MSSC Research Grant Application (MSSC-1), if there are co-applicants for the grant, the principal applicant and all co-applicants should place their Common CVs within the "CV" folder. All CVs must be in PDF format.

Budget
When requested, this folder contains all information pertaining to the application's budget requirements. This includes a spreadsheet, a written justification, and electronic copies of quotations or information that the reviewers may find helpful when assessing the application's proposed budget. All materials must be in PDF format whenever possible.

Progress Report
When requested, this folder contains a detailed description of the progress made by the applicant in their research. The submitted files must be in PDF format.

Other Grants
When requested, this folder contains information relating to other grants that the applicant holds or for which they have applied. The information for each grant held by the applicant should be placed in a clearly labeled subfolder within the "Other Grants" folder and should include both a summary of the research proposal and a budget page. Again, each file must be in PDF format.

Appendix
This folder contains all supplementary material that may be useful to the reviewers in assessing the application, including images, copies of research papers, and presentations made by the applicant (ex. PowerPoint). These files should be clearly referenced in the application form itself. All documents should be in PDF format whenever possible. Applicants may also place html format files (web pages) in the Appendix folder. These html files may be used to create interactive presentations of Appendix material reviewers. Any digital video clips placed in the Appendix folder must be in Quicktime format (.avi or .mov). Quicktime is freely available for Mac and Windows operating systems from www.quicktime.com.

Note: The application form itself (designated MSSC-x, where x is a number between 1 and 6) should be included in the main (or root) directory. When the main directory is opened, therefore, the reviewer should see as many as five folders and the completed application form only.

Converting Files to PDF

Converting a Word document (.doc extension) or an Excel spreadsheet (.xls extension) to a Portable Document Format file (.pdf extension) [for Microsoft Windows]

Note: For these instructions to work, the full version of Acrobat must be installed.

  1. Open the document to be converted in the appropriate application (ie. MS Word for .doc files or MS Excel for .xls files).
  2. Select "Print…" from the "File" menu.
  3. Choose the PDF printer from the pulldown "Name:" list.
  4. Click on "OK" to save the PDF file.
  5. Specify the name for the file and the location where it is to be saved, making sure to use the .pdf extension (ex. MyDocumentName.pdf).

Converting a Word document (.doc extension) or an Excel spreadsheet (.xls extension) to a Portable Document Format file (.pdf extension) [for Mac OS X]

  1. Open the document to be converted in the appropriate application (ie. MS Word for .doc files or MS Excel for .xls files).
  2. Select "Print…" from the "File" menu.
  3. Select "Save As PDF…".
  4. Specify where the document is to be saved in the "Where:" field.
  5. Specify the name for the file to be saved in the "Save As:" field, making sure to use a .pdf extension (ex. MyDocumentName.pdf).
